Thyme Care is seeking an Oncology Nurse - Transitions of Care to serve members diagnosed with cancer, providing crucial support throughout their oncology journey. In this remote role, you will conduct clinical assessments, coordinate care transitions, and educate members and caregivers over the phone. The ideal candidate will have an active, unrestricted compact RN license, at least 5 years of nursing experience with 3 years in oncology or case management, and be a Certified Case Manager (CCM). This position is perfect for a compassionate, organized nurse who is passionate about improving the cancer care experience and comfortable with new technologies.
